The form must also meet the following requirements:

The phone-number field may be optional.
If the phone-number field is required, SMS consent must be provided through a separate optional checkbox.
Visitors cannot be required to consent to SMS messaging in order to submit the form.
The consent checkbox cannot be preselected.
